Maybe this only happens to me.

Every now and again when I enter the ladies restroom, I'll be thinking about something, and not paying a lot of attention to where I'm going. A few times now I've walked in there with my mind somewhere else, and suddenly spotted a pair of really masculine shoes inside one of the stalls. Everytime this happens, I have a few seconds where I panic and think to myself "Oh no! Am I in the men's room?"

After a second my brain will sort of rejoin me, and I can logically ascertain from the presence of a vending machine, and the absence of any urinals that I am actually in the ladies room, despite that pair of shoes someone has decided to wear. Maybe this never happens to other people? For the record, I have never actually wandered into the wrong restroom. I've just had a few false alarms.

I've had this experience, but it's usually not shoes or anything that trigger it. I will just breeze through a restroom door and just as the door is closing behind me, I'll have that intrusive thought -- the "Did I even look at the sign on the door?" thought. It happens mostly in strange places, but sometimes even in restrooms I've frequented routinely. Very disconcerting.
